Role Purpose
To provide practical, short-term support to people returning home from hospital, helping them remain safe, independent and connected to their local community during their recovery.
Main Responsibilities
- Deliver food, essential items and Community Support Discharge Packs to service users across the borough using your own vehicle. Mileage expenses will be reimbursed in accordance with Brentwood CVS's volunteer expenses policy
- Signpost service users to relevant community and voluntary services.
- Record activities and report any concerns to the Hospital Discharge Support (HDS) Coordinator.
- Follow all Brentwood CVS policies, including safeguarding, confidentiality and data protection requirements.
Boundaries of the Role
- Volunteers do not provide personal care or medical support.
- Volunteers do not enter service users’ homes. All deliveries and contact take place at the doorstep or another agreed safe external location.
- Any safeguarding or welfare concerns must be reported immediately to the HDS Coordinator or Director
- No cash handling will take place.

We are looking for volunteers who are:
- Friendly, reliable and compassionate.

- Respectful of confidentiality and professional boundaries.
- Organised and able to follow instructions.
- Committed to treating all individuals with dignity and respect.
- Proof of motor insurance that covers volunteer driving activities (most insurers do not charge an additional premium for this, but volunteers should check with their insurer).
