Speaking to patients who have recently been discharged from Colchester General Hospital in regards support they might require now they have left hospital, including getting: Medication or Food services for them. Befriending and telephone support. Fall prevention slippers. Winter Resilience Packs. Walking Buddys. Social and Green Prescribing. Induction, Training, Resources and ongoing Support given. You do not require any medical knowledge. Your help and support of some of the more vulnerable citizens will in NE Essex will make a great difference to the hospital and to your local community.
